KIDS Enrollment

To enroll with KIDS Plus you need to complete the KIDS Plus IIS Clinic Enrollment Form. Use the following steps to do this.

1. Go to https://kids.phila.gov/index.php/programs/kids/meaningful-use/.
    

2. Click the Provider Enrollment link.
    

3. Click the KIDS Plus IIS Clinic-Health System Enrollment Form link.
    

4. Fill out the form online, checking in Part IV the HL7 Messaging option with the HL7 Version of 2.5.1, and Method of Batch Unidirectional through SFTP.
   
   NOTE: You can also print the blank form and fill out by hand.
    

5. When finished, email the completed form to KIDSRegistry@phila.gov, or print and fax the form to: (215) 238-6944.
   
   NOTE: You should save the file prior to emailing it, and also for future reference.
