Discontinue a Medication in a Chart Note

Use the following steps to discontinue a medication for a patient in a chart note.

1. Open an existing or new Chart Note for the patient.
    

2. Click the Medication button, then select Discontinue Medication, and then select the Medication you want to discontinue for the patient.
   
   
   
    

3. In the End Medication dialog, select a Reason why the medication was discontinued for the patient.
    

4. In the End Date field, modify the Date and Time as needed. These values default to the system date and time.
    

5. In the Comments field, enter a free-text Comment, as needed.
    

6. When finished, click the OK button. The system will then append the note with the discontinuation and reason for the discontinuation (if configured) and also update the patient's History tab with this information.
