Patient Empanelment Report

The Patient Empanelment Report allows you to easily determine and view patients that are empaneled within the practice, as well displaying an empanelment rate, to help you meet CPC+ requirements. A patient is considered empaneled when a Principal Care Provider is entered for the patient in the ID tab in Clinical, or a Provider is entered for a patient in the Patient screen of Practice Manager.

 

Use the following steps to access and run the Patient Empanelment Report.

 

 

  1. To access the Patient Empanelment Report, click Reports > Patient Empanelment Report.
     

  2. In the Filters section of the Patient Empanelment Report, modify the Appointment Date From and Appointment Date To, and then select the Practice(s), as needed.

    NOTE: If there is only one practice in your system, it will default in Practice field. If there are multiple practices in your system, the Practice filed will be blank. You can select multiple practices, if needed.
     

  3. You can then select any Appointment Reasons, an Empaneled Provider, Practice Manager Patient Statuses, Appointment Providers, or  Risk Stratification items in the corresponding fields, if applicable.
     

  4. After the applicable filter items are selected, click the Run Report button to generate the report for the filter criteria selected.
     

  5. After the report has been generated, you can sort the report by clicking any of the column headings in the Results area. An Empaneled Rate percentage will be displayed in the bottom left of the dialog, which is the percentage of empaneled patients out the total patients returned for the selected filter criteria.



     

  6. Click the Save to File button, if you would like to save this report to a file, which can then be exported into Excel and/or printed.

    After the Save to File button has been clicked, a Save As dialog will appear allowing you to select the file location for the file. After the location has been selected, and the File name modified as needed, click the Save button.

    The file will then be saved and you will return to the Patient Empanelment Report.



     

  7. When finished, close out of the Patient Empanelment Report dialog.
