Use the following steps to add a patient representative for a patient.
From the patient's open chart, click the ID tab.
In the Patient Representative section, click the
Add button.
In the Patient Representative dialog, select the
Type (Emergency Contact, Health
Care Proxy, Legal Guardian, Next of Kin, Primary Caregiver, Patient Portal
Representative, etc.) of representative you are adding. You can select
multiple types for a representative. Likewise, you can create additional
Types via the Edit button.
Enter the First,
Middle Initial, and Last
Name for the representative in the corresponding fields.
Enter the Address,
City, State,
and Zip Code for the representative.
Select the Relation
to the patient for the representative.
NOTE: Additional relation types can be added by clicking the adjacent
Add button.
Enter any applicable Phone
Numbers for the representative in the corresponding fields.
The Patient Portal section, as well as the Patient
Portal Representative option in the Type drop-down list, will be accessible
for those patient's that are registered for the PatientPortal. In this
section you have the ability to enter a valid Email
address for the patient representative. An email address must be entered
in the corresponding field for the patient representative to be authorized.
NOTE: When entering an email address, if it is not entered correctly,
a warning message will appear indicating the email address is not in the
correct format. A valid email format will include an at symbol "@",
with valid text appearing before and after the @, a period ".",
and an extension following the period (i.e., johndoe@hotmail.com).
When finished, click the OK
button. If the Patient Portal Representative type was selected, a Patient
Portal Authorization dialog will appear confirming access to the patient's
patient portal account. Click the Yes
button to authorize the patient representative.
After the Yes button has been
clicked, the system will send a registration email to the address in the
Email field, and the representative
will then appear in the Patient Representative list.
Once authorized and registered for the PatientPortal, that patient
representative will then have access to a clinical summary for each office
visit; be able to view, download, add, modify, and/or transmit health
information via the PatientPortal; as well as send health related messages
to the provider. In other words, the patient representative will have
full access to the patient’s PatientPortal account, just as the patient
does.
